Survey Overview

You are taking part in a project that is funded under the Ards and North Down Borough Council’s PEACEPLUS Action Plan. This is funded by the Special EU Programmes Body. They require Council, and all the funded projects in the Action Plan to collate some equality and monitoring information.

Thank you for agreeing to complete the Registration and Equality Data Monitoring. This survey is to be completed at the beginning of the project.

It will take approximately 5 minutes to complete.

Your responses will help measure and evidence the project’s reach and impact and improve its design and delivery in the future. They will also help the body responsible for managing the funding (SEUPB) ensure that the projects delivering the PEACEPLUS Programme are effective, fit for purpose and contribute to the region’s peace and prosperity.

The law protects your rights to privacy and ensures that your personal data is not shared unnecessarily. For further details regarding the types of personal data SEUPB collects, how it is used, with whom it is shared, and the rights available to you as a data subject regarding this use of your information, please refer to the Privacy Notice at https://www.seupb.eu/privacy-notice.

If you would like to read Ards and North Down Borough Council’s Privacy Notice it is available to read online at: https://www.ardsandnorthdown.gov.uk/privacy-and-cookies

The organisation directly delivering your project will have its own policies and procedures on data protection. Please ask them directly about that if you need any more information as they will be the key organisation holding your sensitive data.

SECTION 2 – EQUALITY DATA MONITORING

Racial or Ethnic Group

A person’s racial or ethnic identity may depend on several factors including nationality, colour, descent, religion, language, culture, and traditions. To reflect this complexity, we ask two different questions.

Health Problem or Disability

This question is about your general health and how it may affect you in terms of taking part in routine or daily activities.  It includes health conditions relating to aging, medical illness, disability and/or dependence on physical aid devices such as hearing aids or mobility aids. Choose ‘Yes, limited a lot’ if you rely on support from others such as family, friends or home help services to carry out day to day activities (like dressing, eating, washing, walking, shopping, travelling or leaving your home to attend appointments or partake in leisure activities).

In this question, a person with caring responsibilities (a carer) refers to someone committed to providing regular (daily, or several times a week) unpaid care to a dependent child, relative, friend, or other person who could not cope without their support.

Thank you for providing this information. We will use this data for monitoring and evaluation purposes only. Make sure to click on  the submit registration button below to complete your registration.
